This Refer A Friend promotional program is being conducted by DrBicuspid.com, located at 1350 N. Kolb Road #215, Tucson, AZ 85715. Eligibility: All active registered members are eligible to participate in this program. Registered member means that you have signed up and completed the membership form for DrBicuspid.com. This giveaway is limited to the United States only. International members are not eligible. Employees who work for the listed charities and their family members are not eligible to participate. Those being referred must be dental professionals who are actively involved in the practice of dentistry (dentists, hygienists, dental assistants, specialists, educators, consultants etc.). Promotional Period: The promotion begins at 12:01 AM PST on March 10, 2011, and ends at 11:59 PM PST on June 8, 2011. Donation Limit: DrBicuspid.com will contribute up to $25,000 to the charities that a member designates at the rate of $10 for every new, referred dental professional who becomes a member. Incomplete or false entries will be void. Referral Benefit: The promotional benefit for referring a new member will be ten (10) dollars contributed to the charity of the referrer’s choice of the listed charities. The contribution will be made on a quarterly basis for the duration of the promotion. General Conditions: This promotion is void where prohibited and is governed by the laws of the United States. All federal, state, and local laws and regulations apply. Fulfillment: Friends you refer to DrBicuspid.com will be sent an e-mail invitation with your member name and e-mail address. The person receiving the invitation must click the “join DrBicuspid.com” button in the e-mail invitation in order for the contribution to be made to the charity. Becoming a member any other way will void the tracking and thus the obligation to make the contribution to the chosen charity. Privacy: The mailing address you supply will not be shared with an outside company. DrBicuspid.com may e-mail at this address if there are questions about any of your referrals.
